Archives Family Search Form

When doing your search, it's best to gather as much information as you can.

To start research you will need:

  • Any copies of documents: birth, marriage, death certificates you may have.
  • If this is not available, a name and approximate date of birth, marriage or death, and an address or a place in Ukraine.
  • Religion of ancestor.
  • Any other information about the ancestors: names, addresses, dates, events.
  • If you do not know the County (Raion) or the Province (Oblast) indicate with a "?" mark. We will look it up for you.
  • There will be additional charges for searching each name from the Archive. The rates may vary depending on the archive from which the information is sought. There is no standard fee structure applying throughout the archive system in Ukraine.
  • This form is designed for a one name search. For additional names, fill out additional forms and submit them.
